What's involved The Palliative Care Clinical Nurse (PC CN) role provides specialist palliative nursing care to individuals with life limiting illness, and their caregivers and family across all care settings within Central West Hospital and Health Service (CWHHS).

The purpose of the role is to achieve optimal palliative care outcomes by identifying, providing and evaluating advanced palliative care interventions for people with life-limiting illness, their caregivers and family based on an ongoing, comprehensive and holistic assessment of the person's health and support needs, personal resources, and personal goals and wishes.
