About the Role

  • Deliver contemporary level of profession specific clinical services assessment, treatment and evaluation, within a multidisciplinary team in accordance with prescribed professional and ethical standards and applying clinical protocols including documentation.
  • Participate as a member of a mental health team in providing comprehensive mental health assessment, case management, intervention or group work as required.
  • Within a multidisciplinary framework undertake routine tasks, ensuring the implementation of individual assessment, diagnosis, treatment, planning Community Team West care delivery to consumers via overall support plan and daily support plan, follow-up, multidisciplinary consultation, liaison with referring agents, discharge planning and evaluation of care using appropriate outcome and data measurement systems.
  • As experience builds, apply increasingly independent clinical judgement to more complex decision making under the clinical practice supervision or professional guidance or more senior staff.
