An exciting opportunity exists for an Aboriginal and Torres Strait Islander Senior Health Worker, the role functions within the Preventative Health Pathways program, which is a multidisciplinary team that promotes integrated service provision across the continuum of primary health care, consistent with principles of client self-management and Hospital Avoidance.

Key Responsibilities include:

  • Advanced understanding of human resource management processes and the primary healthcare practice framework. Provide supervision, mentoring and training to the Aboriginal and Torres Strait Islander Health Workers.
  • Prepare and distribute high quality data reports focusing on activity and outcomes for service provision as directed by the Service Manager. Maintain accurate and timely documentation through the provision of monthly activity reports.
  • Deliver evidence-based care by working collaboratively and in partnership with the Aboriginal and Torres Strait Islander community, consumers, health service providers and other health professionals.

Remuneration value comprising salary of $3434.30 - $3622.70p.f., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HW6) (Fixed term temporary full time 76hrs p.f.).
